Children from birth through age 20 with medically complex conditions that require continuous care including but not limited to: Cerebral Palsy, Chronic Asthma, Stroke, Diabetes, Epilepsy, Congenital Heart defects, Medication Monitoring, Down Syndrome, Tube feeding, Colostomy care, Spina bifida.
